Abstract
Sağlık Hizmet Sunucuları önemli ekonomik büyüklüklere sahiptir. Ülkemiz bir sosyal devlet olduğundan ve nüfusun çoğunluğu Genel Sağlık Sigortası kapsamında olduğundan sağlık hizmetleri büyük oranda Sosyal Güvenlik Kurumu (SGK) tarafından finanse edilmektedir. Kamuya ait fonların daha etkin ve verimli kullanımı gerekmektedir. Bunun için işletme maliyetlerini sıkı bir şekilde takip ve kontrol altına alıp yurtdışında uygulanan en ileri maliyet muhasebesi yöntemlerini ve yazılım araçlarını kullanmak suretiyle, ancak devletin şart koştuğu kalite standartlarından ödün vermeden, hizmet vermeleri gereklidir. Maliyet muhasebesi konusunda çalışan mevcut yazılım araçlarının tamamı yurtdışından tedarik edilmektedir. Hem de bu ürünlerin kullanılmadan önce Ülkemiz şartlarına uyarlanması gerekmektedir. Ayrıca hem yıllık lisans maliyetleri hem de bakım-idame-iyileştirme maliyetleri astronomik düzeydedir. İhtiyaç duyulan yazılım-tabanlı sistemleri yerli imkanlarla geliştirip, safhalar halinde sağlık hizmet sağlayıcısı işletmelerinin hizmetine sunulması özel bir önem taşımaktadır. Son yıllarda zamana bağlı maliyet hesabı sistemi gelişmiştir. Bu gelirlerin giderlere oranı ile yapılan hesaplamalar ile kıyaslandığında çok ileri bir yöntemdir. Ancak bu yöntemin çalışması için NiT(Nesnelerin interneti) kullanımı gerekliliktir. Günümüz sağlık hizmet sunucularının NiT olarak kullanabileceği yol ve yöntemler ise her geçen güm artmaktadır. Nesnelerin interneti ve zamana bağlı maliyet muhasebesi kullanımı konusunda duyulmuş yaygın örnekler yoktur. Kurulması önerilen bu çeşit bir sistem tasarım aşamasında olacağından bu makale aynı zamanda ilk tasarımlar için rehber olacaktır. En ileri ülkelerde bile ancak az sayıda firma tarafından pazara sunulan ve gelişmiş yöntem ve teknolojileri kullanan bu nitelikte bir ürün bulunmaktadır. Makale, sağlık hizmet sunucusu işletmecilerinin devlet tarafından ortaya konulan sağlık kalite standartlarına sadık kalmak suretiyle sağlık hizmetlerini en optimum şekilde verirken aynı zamanda da yine devlet tarafından ödenecek hizmet bedelleri dahilinde sağlık hizmetlerini en karlı şekilde verebilmelerini sağlayacak bir hassas maliyet analizi sistemi geliştirilmesinin ilkelerini ortaya koymaktadır.

Healthcare Service Providers have significant economic sizes. Since our country is a social state and the majority of the population is covered by General Health Insurance, health services are mostly financed by the Social Security Institution (SGK). Public funds need to be used more effectively and efficiently. Hence they must strictly monitor and control operating costs and provide services by using the most advanced cost accounting methods and software tools applied abroad, but without compromising the quality standards required by the state. All of the existing software tools working on cost accounting are procured from abroad. Moreover, these products must be adapted to the conditions of our country before they are used. In addition, both annual license costs and maintenance and improvement costs are at an astronomical level. It is of special importance that the required software-based systems are developed with local resources and presented to the service of health service provider enterprises in stages. Time-driven cost calculation system has developed in recent years. This is a very advanced method when compared with the calculations made with the ratio of income to expenses. However, the use of NiT is necessary for this method to work. The ways and methods that today's healthcare providers can use as NiT are increasing day by day. There are no common examples of the use of NiT and time-driven cost accounting. This article will also be a guide for initial designs, as such a system proposed to be installed will be at the design stage. Even in the most advanced countries, there is a product of this nature that is marketed by only a few companies and uses advanced methods and technologies. The article reveals the principles of developing a sensitive cost analysis system that will enable health service provider operators to provide health services in the most optimal way by adhering to the health quality standards set by the state, while at the same time providing health services in the most profitable way within the service costs to be paid by the state.
